How does a steam vacuum breaker work?

On steam-to-water heat exchangers, vacuum breakers should always be used. Is that correct? The steam trap is closed when steam is present in the shell. The heat exchanger shell is filled with steam when the temperature control valve closes. A vacuum is created when steam condenses in this closed chamber.
