What causes excessive crankcase vacuum?

The PCV valve was taken out of the valve cover and replaced with a flow meter. The engine ran at both high and low speeds after the fresh air inlet was plugged. Is that correct? If the fresh air inlet becomes restricted or the wrong PCV valve is used, excessive crankcase under-pressure can occur.
