Why do GFCI outlets go bad?

A GFCI can be mistaken or failed in three ways. The first way it can fail is that it does not fall when the test button is pressed and the output remains hot or dead. The second way is that the travel button remains out of service and will not restart and the third, and the most dangerous, is that the button falls, but the output remains hot.
